Cardiac Conditioning
Clove Lakes is particularly appropriate for cardiac patients as it
provides a critical link in the recovery process. Based on the concept
that a supervised rehabilitation program is the key to a return to everyday
activity, patients referred to Clove Lakes receive an individually-tailored
exercise program, based on medical history and overall functioning.
Exercise sessions are conducted and supervised in our gym by specially trained therapists and a physiatrist. We provide the most appropriate care possible to ensure a safe recovery. As a consequence, every hospital on Staten Island as well as other elite hospitals in the greater metropolitan area admit their cardiac patients to Clove Lakes.
Working Together For You
Cardiac Conditioning begins upon admission with the average patient participating
in rehab five days a week. A physiatrist, a physician trained in the
diagnosis and treatment of neuromusculoskeletal diseases and injuries, works
with other members of the Interdisciplinary Team - physicians, nurses, physical
and occupational therapists, speech pathologists, psychologists, recreation
therapists, dietitians, and social workers - to tailor an individualized treatment
plan. For patients confined to a wheelchair, or for those with other
physical limitations, an exercise program is designed to work with the disability.
Who Can Participate?
Cardiac Conditioning is designed for patients who need continued skilled
care and rehab following surgery or illness. We have successfully helped
patients who are recovering from:
Medical
Clove Lakes Health Care and Rehabilitation Center is dedicated to
providing the highest-quality medical care for our residents. The Medical
Staff, consisting of 70 board certified physicians in 24 medical specialties,
is available 24 hours a day / 7 days a week.
Nursing
Registered professional nurses are on duty 24 hours a day to coordinate
the delivery of resident care. Our nurses hold certifications and/or specialized
training in geriatrics, psychiatry, rehabilitation, infection control and
IV therapy. In addition, a full-time psychiatric nurse, rehab nurse, infection
control nurse and in-service department provide continuing education to
keep staff up to date on the latest clinical technology and skills. Clove
Lakes Health Care and Rehabilitation Center is able to deliver skilled
nursing care services which include, but are not limited to, IV therapy,
respiratory care, tracheostomy care, tube feedings, peritoneal dialysis,
wound care, TPN and the care of the terminally ill.
Rehabilitation
The focus of Rehabilitative Services at Clove Lakes is on promoting
recovery and returning patients to functional independence as quickly as
possible. We have achieved the very highest return rates to the community
through early intervention and aggressive treatment.
Hospice Care
The Hospice Program affirms life. It is our express goal to enable skilled
nursing facility residents in the advanced stages of illness to remain
in a familiar and supportive environment while receiving relief from pain
and control of symptoms.
Social Services
Our dedicated and skilled social service staff serves as advocate and advisor
to the resident and their family. To ensure a smooth transition and adjustment
to placement, a social worker is assigned to each resident. Family members
can also be accommodated during evening hours. Social Services also coordinates
the staff language bank to assist residents, whose primary language is not
English, in making their needs known.
To facilitate discharge back to the community, our social workers assist patients and their families in the development of a comprehensive treatment plan, developed on the day of admission. Weekly support and information groups are provided prior to and after returning home.
Recreation
Therapeutic recreation programs are available to all patients and residents,
including large and small groups, 1:1 activities, special entertainment,
movies, holiday events, birthday parties, and trips. At Clove
Lakes, we offer Pet Therapy and Music Therapy (click for more information on each).
Dietary
Selective menus empower residents to make their own choices. Residents participate
in menu planning through our monthly Menu Committee Meetings, although diet
restrictions based on diagnosis may prevail.
Our newly-renovated kitchen uses state-of-the-art trayline and tray systems to promote quality and efficiency in meal service.
Pastoral Care
At Clove Lakes, interdenominational religious services, available
through The Pastoral Care Program, play a key role in giving residents
a sense of community. In addition to bringing in religious leaders
who conduct worship services in-house, Clove Lakes provides counseling
for all religious traditions. The presence of our chaplain, Sister
Mary Murtagh, OSF, weaves spirituality into day-to-day life. The
Program allows residents of all faiths to reconnect with their faith through
daily Mass, communion or fellowship services.
Jewish residents join Rabbi Morris Zachariash each Friday as he conducts a centuries-old tradition of lighting the Shabbat candles and saying the blessings for those present and deceased. Kosher wine and challah is provided while friends and relatives alike are invited to join in the celebration. Residents who are frail receive a visit from the Rabbi.
A newly redesigned interfaith Chapel affords residents the opportunity
to deepen their relationship with God in a specialized setting, says Sister
Mary. Since its founding more than thirty years ago, the Pastoral
Care Program has helped thousands of residents find a sense of joy in what
life provides.
