The Admission Process
Upon admission, each resident is evaluated by our interdisciplinary
team. The team develops a comprehensive care plan which includes
all medical, nursing, rehabilitation,
nutritional, psychological and therapeutic needs. The resident, along
with family members, actively participates in the planing process.
Each care plan is individualized, taking into account a resident's likes
and dislikes, hobbies, interests and functional capabilities.
Helping the Family With Special Needs
Our dedicated social service staff serves as both an advocate and advisor
to the resident and family members. We are well aware of the difficulty
residents and families have when long term care placement becomes a necessity.
Because of this difficulty, social workers make the transition period their
top priority. A social worker is assigned to each resident on the
day of admission. This individual is also available to assist family
members.
Rehabilitation
Provides State-of-the-Art Therapies
Each resident is evaluated upon admission for rehabilitation services
which include physical, occupational and/or speech therapy. Based
on the residents' needs, a plan of care is developed and carried out by
a team of therapists. Our goal is to assist each resident to maintain his/her
optimum level of functioning.
Vascular Wound Management
Vascular wound care management is coordinated by the vascular surgeon
who makes biweekly rounds at Clove Lakes. Collaboration and
communication between all members of the vascular team, including the resident's
physician, nurses and therapists regarding the medical regimen, assures
speedy recovery on a wide spectrum of conditions.
