Long Term Skilled Nursing Care

Clove Lakes has specialized in delivering quality long term care to residents since we first opened our doors in 1967.  Upon admission, each resident is evaluated by our interdisciplinary team.  The team develops a comprehensive care plan which includes all medical, nursing, rehabilitation, nutritional, psychosocial and therapeutic needs.  The residents, when possible, and family members actively participate in the comprehensive care planning process. Each care plan is individualized, taking into account a residents' likes and dislikes, hobbies, interests, and capacities.


Our dedicated and skilled social service staff serves as both an advocate and advisor to residents and their families.  We are well aware of the difficulty many families have when long term care placement becomes a necessity.  The social workers make the transition adjustment period for families and residents their top priority.  A social worker is assigned to each resident on the day of admission.  The social work staff is always available to discuss needs or concerns and have evening and weekend hours available to accommodate family members. 


Each resident is evaluated upon admission, based on need, for rehabilitative services in physical, occupational and/or speech therapy.  Thereafter, intensive, restorative or maintenance therapies are carried out by a team of therapists in conjunction with the resident's plan of care.  Our goal is to assist each resident with the means to maintain his/her optimum level of functioning.
